This position provides individualized teaching, training, and support needed to adults with severe and persistent mental illnesses to help them maintain their independence in the community.
Case Coordinators will work with clients in their homes and throughout the community to provide life skills training and case management services.
This role assumes responsibility for advocating for, coordinating the delivery of, and monitoring a wide variety of services and supports for eligible persons to enable people to lead meaningful and productive lives.
Additionally, you will be working as part of an interdisciplinary team with opportunities for cross-training, development, and clinical supervision with the option of becoming a Certified Psychiatric Rehabilitation Practitioner (CPRP).
This role allows for a flexible schedule.
A high school diploma and two years relevant experience in the Behavioral Health field required.
Valid driver's license required.
Must have a personal vehicle to use for work.
Must Complete a Background Check and Drug Screening Must be able to work on-call as needed All positions at Humanim are to encourage supported employment for those people we serve who have expressed an interest in such employment, including consumers with long term mental illness.
To that end, every employee will promptly refer any consumer with an interest in obtaining supported employment to appropriate Vocational Staff and will actively participate as needed in the Evidence-Based Practice in supported employment initiative/model.
