is currently seeking a temp or temp-to-perm, full-time Social Service Associate to provide services to residents in long term care facility in Washington County, Maryland.
Responsibilities include (not limited) psycho-social assessments upon admission; assess emotional and health related needs; discharge planning; provide educational information and resources; collaborate with physician and nurses on care plans; and knowledge and understanding of current Federal and State laws and regulations in accordance to skilled nursing facility.
Position Requirements Candidates must be Licensed Bachelor of Social Work.
1 year of human services preferred.
