Nurse Practitioner - CRNP
Johns Hopkins Community Physicians (JHCP) is looking for a Nurse Practitioner to join our Howard County Family Practice new SHARP model.
SHARP (small high acuity/risk panel) is a Nurse Practitioner led small panel primary care program.
Johns Hopkins SHARP providers co-manage a small panel of high-risk adult patients in a primary care practice, enhancing standard PCP care.
The SHARP Nurse Practitioner collaborates with 3-5 established PCPs to provide enhanced, high touch, independent and collaborative primary care to a select group of high-risk patients.
SHARP NP enjoy extended visits and significantly lower visit volumes than seen in standard primary care practices.
Johns Hopkins Community Physicians serves Maryland, Virginia and Washington DC with over 40 locations.

JHCP Howard County Practice, Columbia, Maryland Full-time, 40 hours per week, 8am to 5pm CRNP License in the State of Maryland required.
CPR Certification required.
